Signature visit counts, communication frequency, and fees are determined in a written proposal. This is not unlimited nursing access or around-the-clock coverage.
Important Information About Our Memberships
Every Bradley Health membership begins with a complimentary introductory phone consultation followed by a $300 Initial RN Consultation & Comprehensive Assessment.
Memberships include the scheduled visits, communication, and care management services defined in the selected plan. Additional skilled nursing procedures, extra visits, medical supplies, appointment accompaniment, and services outside the membership scope may incur separate fees.
Bradley Health provides scheduled, personalized nursing and care management services. Memberships do not include emergency response, continuous supervision, or 24-hour on-call nursing. All services are subject to clinical appropriateness, availability, and applicable requirements.
Skilled Nursing Services
Additional Skilled Nursing Services
Bradley Health offers individualized skilled nursing services for clients requiring care beyond routine nursing assessments and care management. Services may include wound care and dressing changes, urinary catheter care, injections, and other clinically appropriate nursing procedures.
Skilled nursing procedures are billed separately from monthly memberships. Fees reflect the nursing service, clinical complexity, time required, and applicable medical supplies. Whenever possible, Bradley Health coordinates the necessary supplies and provides transparent pricing in advance, creating a convenient, seamless experience for patients and families.
Services are subject to clinical assessment, appropriate medical orders, availability, and applicable practice requirements.

- Services involving vascular access, infusion therapy, wound VACs, or other specialized procedures would require additional competency, order, insurance, and regulatory review before being offered.
- Supplies- Bradley Health will provide supplies and additional cost of supplies will be billed.
- Each procedure price covers one local home visit, the specified nursing task, and routine documentation, within a defined time limit.
- Standard and specialty supplies are charged separately, with the full price explained before the visit.

Additional procedures during the same visit receive an individualized combined quote rather than automatically charging two full visit fees.

Longer visits, significant travel, and after-hours appointments may carry additional fees, disclosed in advance.
